About the Role
The Health Visitor will provide a health visiting service to all children and their families based on the national refreshed health visiting model and evidenced by the Healthy Child Programme 0-5 years (HCP). The role involves developing health promotion and public health programmes in accordance with identified needs, local and national directives. You will develop, implement and evaluate health plans in response to the needs of individuals, families, and the community in partnership with clients. The role requires preparing reports and participating in child protection conferences, Team Around the Family meetings, Neighbourhood Acting meetings, court attendances, and multi-agency planning meetings as required.

The service is committed to improving the lives of children and young people in the borough through a fully integrated 0-19 health and social care service.
Experience
Experience working as a Health Visitor within community or public health settings is essential.

- Specialist Community Public Health Nurse Qualification
- Registered Nurse prescriber
- Evidence of continuous professional development
- Ability to assess effectively, lead teams, manage performance and risk, and plan in an outcome-focused way
- Commitment to sharing learning and collaborating with practitioners and multi-agency partners to achieve outcomes for children and families
- IT literate, including use of clinical systems, word processing, email, and internet
