About the Role
Join Thames Reach as a Lead Worker within the City Outreach team, where your understanding, compassion, and commitment to supporting people experiencing street homelessness will be highly valued. Thames Reach delivers vital services across London to help end homelessness by working directly with people sleeping rough and those in hostels, preventing homelessness, and supporting individuals to rebuild their lives through access to work, training, and volunteering opportunities. The Outreach teams operate flexibly and respond rapidly to reach people sleeping rough, assessing their needs and creating tailored support plans. These plans include obtaining identification, accessing health services, and referrals to specialist agencies, including immigration support.

As a Lead Worker, you will work closely with the Senior Practitioner and Lead Manager to ensure effective and assertive support for people sleeping in encampments. You will lead outreach shifts, assess new clients, build trusting relationships, and manage a caseload of rough sleepers, identifying the quickest and most appropriate routes off the street. Your role will involve dynamic risk assessments, safeguarding work, and using databases and virtual tools to map rough sleeping hotspots and encampments to target support effectively. You will represent the team at multi-agency meetings and case conferences, leading work around tents and encampments with police and community safety partners.

Thames Reach is proud to be listed among the top 100 Best Workplaces in the UK, offering a rewarding environment to develop your skills and build a fulfilling career.
