Hours
One full time position (37.5 hours per week) and one part time position (24.75 hours per week)

Salary
£25,760 per annum plus High Cost Area Supplement of 15% (pro rata for part time)

About the Role
We are seeking two Rehabilitation Assistants (Band 3) to join the Waltham Forest Falls Prevention Service. You will support the multi-professional team in delivering high-quality therapy services to patients within the community. Your role will involve assisting with the assessment and implementation of falls prevention treatment programmes and care plans for patients with diverse physical and psychological conditions. You will carry out agreed treatment programmes, sometimes working alone in patients’ homes, and assist in delivering Falls Education and Balance Classes.

Key responsibilities include carrying out prescribed treatment programmes and care plans, monitoring patient progress, reporting to Registered Therapists or Nurses, assisting with home visit assessments and treatments, and working safely as a lone practitioner in community and domiciliary settings. You will be responsible for reporting clinical risks within your caseload and working within Trust and professional clinical guidelines. You will liaise with the multi-disciplinary team and promote patient dignity and choice at all times.

Requirements

Experience
Previous experience in a rehabilitation or community healthcare setting is desirable but not essential. Enthusiasm and a willingness to learn are key.

About you
You should be motivated, compassionate, and able to work independently and as part of a team. You must be confident working in patients’ homes and community settings, with a good understanding of clinical risk and patient safety. Respect for patient dignity and choice is essential.

Qualifications
Relevant healthcare qualifications or experience in rehabilitation support roles are advantageous. You must have a good working knowledge of health and safety principles and be committed to working within Trust clinical guidelines. A full induction and training will be provided.
