Hours
This is a 24-hour resettlement service supporting up to six customers transitioning from hospital into the community. You will work on a rota basis covering shifts between 8:00am to 4:00pm and 1:00pm to 9:00pm. The service operates Monday to Sunday and includes lone working with management on-call support available when required.

We are looking for a compassionate and proactive Resettlement Support Worker to assist people leaving hospital who may be homeless, seeking asylum, or facing significant barriers to housing and stability.

Based in a small, short-stay accommodation service in Stoke Newington, you will play a vital role in helping customers settle into the community, develop essential life skills, and move on to longer-term housing. This is a varied and rewarding role where you will build trusted relationships and make a meaningful difference to people's lives every day.

Your responsibilities will include providing high-quality housing management and resettlement support, helping customers gain the confidence and skills needed to live independently. You will support customers to settle into their accommodation and help them access benefits, healthcare, community services, and other sources of support. You will provide practical guidance on independent living, including budgeting, money management, healthy eating, and maintaining a home.

You will work closely with hospitals, housing providers, support agencies, and community organisations to achieve positive outcomes. Supporting customers to identify and move on to suitable long-term accommodation will be a key part of your role. You will carry out health and safety checks, room inspections, and welfare visits to maintain a safe and welcoming environment. Responding to safeguarding concerns and working proactively to manage risk will also be essential.

Maintaining accurate records, support plans, and risk assessments is required to ensure the highest standards of service delivery.

Requirements

Experience
Experience supporting vulnerable adults within housing, homelessness, resettlement, supported housing, or a similar environment is essential. Knowledge of housing management, welfare benefits, and tenancy sustainment is required. You should have an understanding of safeguarding, health and safety, and risk management. Experience completing support plans, risk assessments, and customer records is necessary. Knowledge of recovery-focused and/or trauma-informed approaches is desirable.

Experience working collaboratively with external agencies and stakeholders is important, along with good IT skills including Microsoft Office and electronic case management systems.

You are passionate about helping vulnerable people rebuild their lives and achieve greater independence. You are patient, resilient, and a confident communicator who can build positive relationships with customers and partner agencies. You are flexible and adaptable, able to respond to a wide range of customer needs, and comfortable working independently and making decisions when lone working.

You are organised, with strong report-writing and record-keeping skills, and able to support customers with practical life skills and independent living.

Qualifications
This role requires an Enhanced DBS check. No specific formal qualifications are stated, but relevant experience and skills as outlined above are essential.
