Description
Job Description 1. Job Summary - To provide assessment, care and implement palliative treatment plans for palliative care at the end of life or in the deteriorating or unstable phase of their illness to support rapid discharge from hospital and preventunnecessary admissions where possible. - To mentor and support assigned nurses and nursing assistants to deliver high quality, safe and effective care on a day-today basis. - To participate in rotation of coordination of shifts. - To work collaboratively with multi-disciplinary Specialist Palliative Care and Community End of Life teams and @Home service to meet the needs of patients referred to the service. 2. Key RelationshipsThese include:Guys and St Thomas Specialist Palliative Care Team, St Christophers Hospice Community Palliative Care Team, District Nurses, Community End of Life Team, Marie Curie, @home Team, Pharmacists, GPs (including SELDOC), Consultants and Hospital Clinical Teams, Practice Development Team(s), Administrative and Clerical Support Staff. 3. Duties and Responsibilities- Support and manage a group of patients in collaboration with the Senior Nurse Practitioner whilst working as part of the multidisciplinary team, delivering individualised and personalised direct patient care overnight. - To effectively hand over to the relevant teams working in the daytime to allow seamless provision of care - Demonstrate appropriate practice and specialist advice in the assessment, planning, implementation and evaluation of patient care - Be responsible for the correct administration of prescribed medication including, management of syringe pumps, enteral nutrition, intravenous therapy etc. - Be responsible for assessing and recognising palliative care emergencies in the specialty. - To interpret information and take appropriate action and to lead others to do the same. - Act as an advocate for patients, to ensure a patient centred approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines - Ensure that patients receive high quality clinical care and a good patient experience, having regardfor their, customs, culture, social circumstances religious beliefs and doctrines. - To undertake holistic assessment and provide interventions such as pain and symptom control, management of subcutaneous syringe pumps, re-hydration, emergency unblocking catheters/catheterisation, supporting management of urgent nursing related issues, intravenous therapy etc - Recognise and avoid situations that may be detrimental to the health and wellbeing of individuals and escalate where necessary - Provide support and guidance to staff in the monitoring planning, delivery and evaluation of individualised care. - Is wholly accountable for his / her practice in line with the NMC code of professional conduct and takes every reasonable opportunity to sustain and improve his / her knowledge and professionalcompetence. - To be responsible, for maintaining a safe environment for patients and other staff and identifying and escalating any concerns about safety when working in the patients home. - Ensure that patient records are stored appropriately. - Support and enable patients and carers to make informed decisions related to their treatment and management. - Ensure that staff take precautions to minimise the risk of cross infection in relation to the safe disposal of equipment and body fluids (including blood). - Assess educational and information needs for patients and families, and devise plans to ensure needs are met. - To be an effective communicator with patients/carers/families about the scope of the Pal@home Service, other services, disease and treatments. - Promote high quality clinical care, assist in the development of the service through the use of evidence based practice and clinical standards.
