About the Role
The Support Coordinator will be part of the Surbiton and Chessington Hub, one of three locality teams in the Royal Borough of Kingston upon Thames. The team supports vulnerable residents under the Care Act 2014, working closely with residents and vulnerable groups to maintain independence and balance support with independent living.

Key responsibilities include:
- Managing and supporting a diverse caseload, including accepting referrals, conducting initial screenings, assessments, and reassessments in line with the Care Act 2014.
- Liaising with external organisations such as GPs, hospitals, health professionals, care providers, and families.
- Implementing care services and undertaking reviews for customers with long-term needs, ensuring statutory timelines are met.
- Handling safeguarding concerns and obtaining financial information to facilitate accurate financial assessments.
- Working within the statutory framework for community care, adhering to council policies and procedures related to assessment, care management, and occupational therapy.
- Ensuring accurate and timely case recording and information sharing in compliance with policies including safeguarding and data protection.
- Promoting early intervention, prevention, customer choice and control, universal services, and social capital building within the community.
- Being financially aware of the cost implications of support planning and advising customers accordingly.
- Working collaboratively and communicating effectively with colleagues and customers.
- Committing to personal performance and development aligned with One Council policies.
- Maintaining confidentiality and complying with relevant policies and legislation.
- Performing any other duties commensurate with the grade of the post as required.

Experience
Experience working with vulnerable adults and knowledge of the Care Act 2014 is essential. Experience in case management, assessment, and support planning within a social care or community setting is highly desirable.
