About the Service and Team
The Kingston Hub is one of three locality teams supporting vulnerable residents under the Care Act 2014. The team consists of 15 members including qualified and unqualified staff, senior practitioners, and a team manager. About the Role and Our Ideal Candidate
As a Support Coordinator, you will manage and support a diverse range of casework including referrals, initial screenings, assessments, and reassessments applying the Care Act 2014 criteria. You will liaise with external organisations such as GPs, hospitals, health professionals, and care providers, work with families, and implement care services. You will undertake reviews for customers with long-term needs, ensure adherence to statutory timelines, address safeguarding concerns, and obtain financial information for accurate assessments. You will also participate in the duty system when required.

You will work rigorously within the statutory framework for community care, adhering to and providing feedback on council policies related to assessment, care management, and occupational therapy. Accurate and timely case recording is essential, along with compliance with safeguarding policies, Equal Opportunities, Health & Safety, and other relevant legislation.

You will promote early intervention, prevention, customer choice and control, universal services, and social capital building within the community. A strong awareness of the cost implications of support planning is required, including advising customers on financial consequences for themselves and the local authority.

Effective communication and teamwork are vital for success in this role. You will contribute to personal and team performance and development, aligned with One Council policies.

Maintaining confidentiality in line with the Data Protection Act and other policies is essential. You may be required to perform other duties commensurate with the grade of the post.

Requirements

Experience
Experience working with vulnerable adults under the Care Act 2014 is essential. Proven ability to manage casework, conduct assessments, and liaise with external organisations is required.
