The Transition to Adulthood Service supports young people aged 16-25 who are preparing to leave care or have already left care. We take immense pride in our role as corporate parents, providing care, respect and support to help young people thrive in every aspect of their lives.

As Team Leader, you will lead a team of personal advisors, providing professional leadership, supervision and management oversight. You will ensure effective pathway planning, safeguarding support and transition arrangements that help young people achieve successful outcomes. You will maintain high standards of practice, support workforce development and drive continuous improvement across the service.

Your responsibilities will include leading and supporting a team of Personal Advisers, overseeing pathway planning, assessments and interventions, ensuring statutory responsibilities under the Children (Leaving Care) Act are met, supporting complex casework and safeguarding concerns, promoting positive outcomes in education, employment, training, accommodation, health and emotional wellbeing, championing young people's voices, building strong partnerships with Adult Social Care, Housing, Health, Education and community organisations, monitoring quality and performance, supporting recruitment and professional development, contributing to service development and promoting a culture of reflective practice and innovation.

Experience
Significant experience within children's social care, leaving care or transition services. Strong knowledge of safeguarding, cared for and leaving care responsibilities and unaccompanied children seeking asylum. Experience of quality assurance, performance management and service improvement. Experience supporting young people through significant life transitions. Strong partnership-working experience across agencies and professional networks.

Qualifications
Relevant professional qualification at degree level or equivalent.
