Hillingdon Council is seeking a committed and skilled Social Worker to join our Adult Safeguarding Triage Team. This rewarding opportunity involves working within a dynamic multi-agency service, playing a vital role in identifying risk, responding to safeguarding concerns and helping adults achieve the outcomes that matter most to them.

Working in accordance with the Care Act 2014 and Making Safeguarding Personal principles, you will support adults at risk through effective screening, assessment and partnership working, ensuring timely and proportionate interventions.

As a Social Worker within the Adult Safeguarding Triage Team, you will be responsible for screening safeguarding concerns, undertaking enquiries and coordinating responses to protect adults who may be experiencing abuse or neglect.

You will work closely with a wide range of professionals and agencies, ensuring safeguarding concerns are managed effectively while placing the adult's wishes, views and desired outcomes at the centre of decision-making.

You will:
- Triage and screen incoming safeguarding contacts and referrals.
- Apply the Section 42 Care Act criteria to determine whether safeguarding enquiries are required.
- Undertake proportionate safeguarding enquiries into allegations of abuse or neglect.
- Engage directly with adults, families, carers and advocates to understand risks and desired outcomes.
- Carry out visits where required to assess risk and gather information.
- Maintain accurate and timely case records and safeguarding documentation.
- Work collaboratively with health services, police, housing providers and voluntary sector organisations.
- Attend and contribute to strategy meetings, risk panels and multi-agency forums.
- Present cases to CMARAC and other multi-agency risk management meetings where appropriate.
- Refer adults to relevant support services, including mental health, substance misuse and domestic abuse services.
- Promote preventative approaches and connect residents with community-based support.
- Contribute to safeguarding best practice and continuous service improvement.

Experience
- Experience working within Adult Social Care and safeguarding services.
- Experience undertaking safeguarding enquiries and risk assessments.
- Experience of effective multi-agency working and partnership collaboration.

Qualifications
- Recognised Social Work qualification.
