About the Role
West London College is seeking two dedicated and experienced Transition Coordinators to join our Inclusive Learning and Support team. Each postholder will have a specialist area of responsibility and work closely with schools, families, local authorities, and College teams to ensure learners receive a coordinated and personalised transition from application through to enrolment, induction, and progression.

The two specialist areas are:
- EHCP and High Needs Learners: Supporting learners with Education, Health and Care Plans (EHCPs) and high needs funding, liaising with schools, Local Authority SEND teams, and other professionals to ensure appropriate provision and transition arrangements.
- Disadvantaged Learners and Additional Support Needs: Supporting learners with identified barriers to learning but without an EHCP, including care experienced learners, young carers, learners with mental health needs, and those from disadvantaged or vulnerable backgrounds.

Key responsibilities include building strong relationships with schools, SENCOs, local authorities, parents, carers, and external partners; coordinating transition activities from enquiry to induction; developing individual transition plans; working with curriculum and support teams to provide appropriate support; monitoring engagement, attendance, and wellbeing; supporting progression into work experience, apprenticeships, employment, and further learning; and maintaining accurate records to contribute to quality improvement and a consistent College-wide transition framework aligned with Department for Education best practice.

Requirements

Experience
Proven experience in coordinating learner transitions, working with schools, local authorities, and external partners. Experience supporting learners with EHCPs, high needs, or additional support needs is highly desirable.

Strong interpersonal and communication skills with the ability to build effective relationships with a wide range of stakeholders. Organised and proactive in identifying and addressing barriers to learning. Committed to safeguarding and promoting the welfare of children, young people, and vulnerable adults. Ability to work collaboratively within a team and contribute to continuous improvement.

Qualifications
Relevant qualifications in education, social care, or a related field are desirable. Commitment to ongoing professional development and training.
