About the Role
West London College is seeking a dedicated and experienced Transition Coordinator to join our Inclusive Learning and Support team. This fixed-term, term-time only position involves specialist responsibility in one of two areas: supporting learners with Education, Health and Care Plans (EHCPs) and high needs funding, or supporting disadvantaged learners and those with additional support needs who do not have an EHCP.

The successful candidate will work closely with schools, families, local authorities, and College teams to ensure learners receive a coordinated and personalised transition from application through to enrolment, induction, and progression.

Key responsibilities include building strong relationships with SENCOs, local authorities, parents, and carers; coordinating transition activities; developing individual transition plans; collaborating with curriculum and support teams; monitoring early engagement and wellbeing; supporting progression into work experience and further learning; and maintaining accurate records to contribute to quality improvement and a consistent College-wide transition framework aligned with Department for Education best practice.

The College is committed to safeguarding and promoting the welfare of all learners and expects staff to share this commitment. Employment is subject to enhanced DBS and other relevant checks in line with Keeping Children Safe in Education guidelines.

We offer excellent employment benefits including generous annual leave, a public sector pension scheme, and great opportunities for continuing professional development (CPD).

Requirements

Experience
Proven experience working with learners with special educational needs or additional support requirements is highly desirable. Experience liaising with schools, local authorities, and external partners to support learner transitions is essential.

About you
You will be a dedicated, organised, and empathetic professional with excellent communication and interpersonal skills. You should be able to build strong relationships with a wide range of stakeholders and work collaboratively to support learners’ individual needs. A commitment to safeguarding and promoting equality, diversity, and inclusion is essential.

Qualifications
Relevant qualifications in education, social care, or a related field are desirable but not mandatory. A willingness to undertake mandatory safeguarding training and other professional development is required.
