About the Service and Team
Adult Social Care is committed to improving care for adults and fostering positive working environments for staff. With new leadership and experienced managers, the service is driving meaningful change to deliver exceptional care that enhances community outcomes. The service operates four Locality teams aligned with Primary Care Network footprints: Wallington, Carshalton, Sutton, and Cheam, supporting people with physical, sensory, and mental health needs through integrated working with NHS colleagues.
In June 2024, the Adults First Contact Service (AFCS) was introduced, comprising a General Duty Team focusing on prevention and immediate needs assessment, and a Safeguarding Triage Team managing risk and safeguarding concerns. This approach ensures timely, skilled responses and collaboration with Locality and Specialist Teams.
