About the Role
As a Community Falls Physiotherapist, you will work closely with patients and their carers to promote self-management strategies that support recovery and independence. You will represent the service at multi-disciplinary team meetings, contributing to discussions on patient care, progress, discharge planning, and ongoing rehabilitation needs. Accurate and timely treatment records must be maintained in line with HPC, CSP, Trust, and departmental standards. You will provide comprehensive progress and discharge reports to GPs, consultants, patients, and other members of the multi-disciplinary team. Managing clinical risk within your patient caseload is essential. Flexibility to provide cover in other clinical areas during staff shortages is required.
Experience
Experience working in physiotherapy, particularly with falls prevention and rehabilitation, is highly desirable.
About you
You are a dedicated and compassionate physiotherapist with excellent communication and teamwork skills. You are able to work independently and manage clinical risks effectively. Flexibility and adaptability to cover other clinical areas when needed are important attributes.
Qualifications
You must be a registered physiotherapist with the Health and Care Professions Council (HCPC) and a member of the Chartered Society of Physiotherapy (CSP). Relevant post-registration experience in falls prevention or community physiotherapy is preferred.
