This programme supports residents who are economically inactive to pursue their life goals and career aspirations. The Trailblazer Life Reworked programme combines skills and employment support, working closely with local GPs, social prescribers, and community services to ensure residents access the right support. Key responsibilities include caseload management, meeting performance targets and KPIs, employer engagement to seek vacancies and promote equality, detailed record keeping using CRM and Microsoft apps, and working with referral partners to identify suitable individuals for the service.
