About the Role
As Lead Practitioner, you will promote a culture of continuous learning and professional development of staff and students across Communities, Adults and Health to enhance the quality of practice and staff wellbeing. You will support the Team Manager by providing clear and responsive practice leadership and supervision, promoting the highest professional standards in social care practice.

Your role includes supporting people to maintain their health, wellbeing and independence by providing access to good quality information and advice, and signposting to local preventative services. You will focus on building resilience and strengths to help individuals stay well and achieve outcomes important to them.

You will provide a high standard of social work to adults and/or carers in line with the Council’s duties under the Care Act 2014 and Barnet policies, procedures, guidelines, and the Code of Practice for social care workers. This includes conducting professional social work assessments with adults and carers, focusing on meeting social care outcomes as outlined in the Department of Health’s Adult Social Care Outcomes Framework.

Safeguarding adults and carers at risk of harm is a key responsibility, promoting Making Safeguarding Personal principles and their wellbeing. You will undertake and review individualised, outcomes-led care and support plans, ensuring wellbeing principles are central to assessment and care planning.
