About the Role
As a Support Worker in our Mental Health Outreach Service, you will be a valued member of a diverse team committed to making a real difference in the lives of people with mental health challenges. With wellbeing at the heart of everything we do, we empower people to achieve their goals and promote independence and emotional wellbeing. You will work closely with colleagues and teams involved in the care of your assigned service users to ensure the best possible service is delivered.
As a key worker, you will identify the needs of service users and collaborate with them to develop care plans focused on life-enhancing outcomes. You will develop respectful, supportive, and sensitive relationships, and assess, plan, implement, manage, and evaluate activities within a person-centered planning framework to help people develop valuable life skills. Ultimately, your role will help improve mental health and wellbeing while enabling individuals to gain greater control over their own lives.

Experience
Some experience in a health and social care role is required. You will have worked with people who have severe and enduring mental health problems, as well as with their families and carers.
About you
You demonstrate an understanding of key principles such as social inclusion, independence, empathy, and empowerment. You are able to plan and deliver varied activities and are comfortable producing written reports and statistical data to demonstrate the impact of your work.

Qualifications
