Shift work on a rota basis covering 8:00am to 4:00pm and 1:00pm to 9:00pm, Monday to Sunday. This is a 24-hour resettlement service with lone working and management on-call support available.

About the Role

We're looking for a compassionate and proactive Resettlement Support Worker to assist people leaving hospital who may be homeless, seeking asylum, or facing significant barriers to housing and stability. Based in a small, short-stay accommodation service in Stoke Newington, you will play a vital role in helping customers settle into the community, develop essential life skills, and move on to longer-term housing. This varied and rewarding role involves building trusted relationships and making a meaningful difference to people's lives every day.

Your responsibilities will include providing high-quality housing management and resettlement support, helping customers gain confidence and skills to live independently. You will support customers to settle into their accommodation and access benefits, healthcare, community services, and other support. You will provide practical guidance on independent living, including budgeting, money management, healthy eating, and maintaining a home. You will work closely with hospitals, housing providers, support agencies, and community organisations to achieve positive outcomes. Additionally, you will support customers to identify and move on to suitable long-term accommodation, carry out health and safety checks, room inspections, and welfare visits to maintain a safe and welcoming environment. Responding to safeguarding concerns and managing risk proactively will also be key parts of your role. Maintaining accurate records, support plans, and risk assessments is essential.

Requirements

Experience
Experience supporting vulnerable adults within housing, homelessness, resettlement, supported housing, or a similar environment. Knowledge of housing management, welfare benefits, and tenancy sustainment. Experience completing support plans, risk assessments, and customer records. Experience working collaboratively with external agencies and stakeholders.

Patient, resilient, and a confident communicator able to build positive relationships with customers and partner agencies. Flexible and adaptable with the ability to respond to a wide range of customer needs. Comfortable working independently and making decisions when lone working. Knowledge of safeguarding, health and safety, and risk management. Understanding of recovery-focused and/or trauma-informed approaches. Ability to support customers with practical life skills and independent living. Good IT skills, including Microsoft Office and electronic case management systems. Organised with strong report-writing and record-keeping skills.

Qualifications
This role requires an Enhanced DBS check. No specific formal qualifications are stated, but relevant experience and skills are essential.
