About the Role
You will work alongside adults of all ages with disabilities, long term illnesses, mental health conditions, sensory impairments, older people and their families with the aim of improving the health and wellbeing of individuals and their communities. The role focuses on reducing demand on adult social care and preventing people from reaching crisis or having their needs escalate.
You will develop partnerships with individuals, families, local organisations and the broader community to promote opportunities for contribution and build a more inclusive community.
Your approach will establish and develop key working relationships and engagement across a range of sectors and communities. You will promote the principle of shared responsibility in the development and delivery of initiatives that improve the health and wellbeing of Barnet residents, working collaboratively with individuals, families, community partners and voluntary and statutory bodies.
This recruitment is for three identical roles; one permanent and two fixed term for 12 months.
Experience
Experience of working with vulnerable adults, supporting them to be as independent as possible. Insight into the difficulties faced by people with escalating or established needs and their families in the wider community is essential.
Confident in working in different community locations, including lone working and visiting residents at home across Barnet, with the ability to travel.
Able to manage a variety of tasks and competing priorities, manage time effectively and work independently.

Flexible and adaptable to working in diverse community settings.
