About the Role
As a Social Care Officer, you will support the delivery of an integrated social care service to an allocated caseload within the agreed performance management, quality assurance, and budgetary framework. Your role will involve providing an effective, efficient, and personalized assessment, support planning, brokerage, and review service to adult service users, carers, and their families. You will ensure that the principles of “Putting People First” are deployed in your day-to-day practice, supporting individuals to have the best possible personalized opportunities and to exercise choice and control to meet their defined outcomes.

Experience
We are looking for a Social Care Officer with experience in case work under the Care Act, including promoting independence and Direct Payments, conducting assessments of needs, reviews, and working with customers and carers in the community.
About you
You should possess good assessment and care planning skills and be passionate about delivering results. Building strong links with community agencies within the Adult Social Care neighbourhood team is essential.
Qualifications
Relevant qualifications in social care are desirable, but a commitment to professional development and a positive attitude are equally important.
