Our Mental Health Adult Social Care Service is looking for a passionate and dedicated Assistant Practitioner to join our amazing team and help make a difference today.
As an Assistant Practitioner within Southwark’s Mental Health Reablement Team, you will play a key role in delivering short-term, targeted interventions that prevent and reduce long-term social care needs while promoting independence and recovery.
You will:
- Support Social Workers and Occupational Therapists to implement tailored support plans
- Work directly with service users to help them achieve their personal goals
- Deliver interventions in the community, at our team base, and in service users’ homes where appropriate
- Promote wellbeing, independence, and recovery through strengths-based practice
- Have opportunities to develop your skills, confidence, and potentially manage your own caseload

About the Team
The Mental Health Reablement Team is a short-term service dedicated to promoting recovery and preventing the need for longer-term social care support. We are a collaborative and supportive multidisciplinary team focused on achieving positive outcomes for residents.

Experience
- Experience supporting service users in a person-centred way
- Experience implementing or contributing to support plans
About you
- Enthusiastic, creative, and committed to person-centred practice
- A good understanding of reablement principles
- Knowledge of the impact of mental health issues on individuals and their daily lives
- Strong communication and interpersonal skills
- Ability to work flexibly across community settings
