About the Service and Team
The Operational Commissioning service area within the Commissioning, Insight & Delivery Division is responsible for brokering services for children and adults with care and support needs. This includes processing invoices from social care providers and supporting individuals who self-commission services via Direct Payments, Shared Lives, and Learning Disability floating support.

About the Role
As a key frontline role within the Brokerage Service, you will arrange specialist care for children, young people, and adult residents with care and support needs, as well as carers. Placements may be in residents’ homes, supported living, care homes (residential or nursing), and other place-based services, including self-funders. Your focus will be on sourcing and securing high-quality placements that align with the outcomes identified in individual support plans.

Requirements

Experience
- Detailed knowledge of Adult Social Care services including residential and nursing care, Children with disabilities services, domiciliary and reablement care.
- Experience delivering service activities and managing caseloads under supervision to meet agreed service standards.
- Proven ability to source care packages including residential/nursing homes, domiciliary care, supported living, respite care, and other place-based services.
- Skilled in negotiating fees with providers to ensure value for money within council budget frameworks.
- Competent in maintaining accurate records and updating care packages on IT systems such as Mosaic.
