Case Management Officer


Location
London Borough of Lewisham. The role is primarily based in Laurence House with a hybrid working approach, requiring regular work both in the office and from home. Visits to service users' homes are also required.
Hours
Part-time, 17.5 hours per week.
Salary
£41,739 to £43,014 per annum (SO1) pro rata. All appointments will be offered at the minimum spinal column point for the grade unless matching current salary with evidential proof.

This is an exciting opportunity for a highly motivated, committed, and performance-oriented individual to join an established team within Lewisham’s model of Integrated Health and Social Care. The role works with multi-disciplinary teams including the voluntary sector to promote independence and personalisation.
As a Case Management Officer, you will assess the needs of vulnerable adults and their carers, support them in arranging services to meet those needs, and review adult social care plans. Accurate recording on case files is a key responsibility.
