About the Role
Join Croydon Council and make a real difference as an Experienced Social Worker within the LIFE Team (Living Independently for Everyone). This permanent role offers the opportunity to work in a dynamic and supportive environment committed to delivering high-quality Adult Social Care services that empower residents to live independently. You will play a key role in strength-based practice, focusing on hospital discharge through the Discharge to Assess (D2A) pathway.

You will undertake strength-based assessments and care planning, provide guidance and informal supervision to Social Workers and Health & Wellbeing Assessors, participate in the Duty Senior rota including work allocation and professional advice, and support colleagues to deliver effective, person-centred interventions. The role involves working collaboratively across multi-disciplinary teams within an integrated, multi-agency service and includes some weekend working as part of a rota system.

Experience
Qualified Social Worker with post-qualifying experience in adult social care and complex casework. Experience in supporting and mentoring colleagues is essential.

Qualifications
