About the Role
As a Social Worker within the Transitions Service (18-25 years), you will support young adults with disabilities, autism, learning disabilities, mental health needs and complex needs as they transition into adulthood. You will adopt a strengths-based, person-centred approach, working collaboratively with individuals, families, carers and partner agencies to promote independence, wellbeing and positive outcomes.
Key responsibilities include undertaking proportionate, strengths-based assessments in accordance with the Care Act 2014, identifying individual strengths, aspirations, eligible needs and desired outcomes. You will work collaboratively with young adults, families, carers and professionals to co-produce outcome-focused support plans that maximise independence, choice and control.
You will provide information, advice and guidance on community resources, services, support networks and relevant legislation to promote wellbeing and self-determination. Developing and maintaining effective multi-agency and multidisciplinary relationships to ensure coordinated and holistic support will be essential.
Additional duties include identifying, assessing and managing risks, ensuring appropriate safeguarding measures and risk management plans are implemented. You will complete Mental Capacity Act 2005 assessments, Best Interests decisions and undertake duties under the Care Act 2014, Mental Health Act 1983 (as amended) and other relevant legislation.
Producing high-quality, analytical and evidence-based assessments, care and support plans, reviews and case recordings in line with statutory and organisational requirements is required. You will promote individuals' rights, strengths, dignity, inclusion and participation within their communities and support young people and their families through the transition from Children's to Adult Social Care, ensuring a smooth and person-centred transition process.

Experience
You will be a skilled and motivated Social Worker with a sound understanding of strengths-based practice and relevant legislation. Experience in assessment, risk management and partnership working is essential.
