About the Role
We are seeking a passionate and skilled Lifelong Links Coordinator to join our Targeted Prevention Hub. This pivotal role supports families to make safe and sustainable plans for their children through a strengths-based, family-led decision-making process. You will work closely with Family Group Conference Coordinators within the Targeted Prevention Hub.
As a Lifelong Links Coordinator, you will work independently to understand the child’s family history and journey into care, exploring who is missing from their life and what the child would benefit from now and in the future. You will facilitate conferences that empower families and networks to reconnect and support the child or young person. Your input will help create child-led plans that resolve concerns, reduce risk, and improve outcomes while keeping everyone safe.
You will be responsible for preparing families, coordinating meetings, and ensuring the process is inclusive, respectful, and aligned with national Lifelong Links and Family Group Conference standards.
Key responsibilities include working closely with social workers, foster carers, and leaving care personal advisors to identify children who will benefit from Lifelong Links interventions; building positive relationships with children and young people; contacting previous foster carers, birth parents, extended family members, and other professionals; facilitating Family Group Conferences; engaging and preparing families and networks; ensuring child-centred planning; providing advocacy and support; maintaining accurate records; and collaborating with social care, education, health, and voluntary sector partners to raise awareness and obtain necessary information.
Experience
- Adequate experience working with families in distress or conflict
- Experience in social work, youth work, community work, early years, mediation, or family support

- Strong facilitation and communication skills
- Commitment to anti-racist practice and cultural sensitivity
- Ability to work flexibly across Brent and occasionally outside normal working hours
Qualifications
- A recognised qualification (e.g. NVQ Level 3 or equivalent) in a relevant field
