Salary
£40,404 - £41,637 per annum (pro rata) (S01). All appointments will be offered at the minimum spinal column point (SCP) for the grade unless matching current salary with evidential proof.

As a Neighbourhood Co-ordinator, you will play a key role in Lewisham’s Neighbourhood Model, promoting integration between health, social care, and community services. You will coordinate multi-disciplinary team meetings, build strong relationships with GP practices and partners, and help shape care pathways for citizens with complex needs. Experience
Proven ability to communicate clearly and professionally, building strong relationships across health, social care, and community partners. Experience in managing multi-disciplinary meetings and priorities effectively is essential. A good understanding of Adult Social Care and community health services is required.
