St Thomas’ has one of the largest critical care units in the UK and one of the busiest emergency departments in London. It is also home to Evelina London Children’s Hospital. Evelina London cares for local children in Lambeth and Southwark and provides specialist services across south east England including cardiac, renal and critical care services. We lead a number of specialist service networks aiming to ensure children are treated locally where possible, but have access to specialist expertise when they need it. Our community services include health visiting, school nursing and support for families of children with long-term conditions. Our adult community services teams deliver care at the heart of the local communities we serve, working in partnership with GPs, local authorities and other healthcare and voluntary sector organisations. Working with our partners in Lambeth and Southwark, we are focusing on new ways of working to improve care for local patients. In February 2021 the Royal Brompton and Harefield joined Guy’s and St Thomas’ NHS Foundation Trust, bringing together world-leading expertise in the care and research of heart and lung disease. We have one of the most ambitious capital investment programmes anywhere in the NHS. Job overview We are currently recruiting for 2 Patient Pathway Navigators to join our Vascular Surgery Department. We are looking for an enthusiastic, dedicated and adaptable individual who has good attention to detail. The role demands excellent prioritisation skills, problem-solving abilities and a collaborative approach to ensure the smooth and efficient management of patient pathway. They will need to work well as part of a team, be professional and personable at all times, able to multi-task as well as being able to work under pressure. Main duties of the job The Patient Pathway Navigator will lead on the tracking of pathways of patients. By working closely with a wide range of clinical and managerial staff, and providing high standard of administrative support, the post holder will ensure patients receive treatment as soon as possible and ensure national targets are met. The post holder will work closely with the Assistant service manager, and Clinical Teams. Act with integrity Detailed job description and main responsibilities - Monitor patients on their pathway and proactively find resolutions to improve the speed of treatment by working directly with clinicians and managers. -Action day-to-day issues, ensuring resolution and escalating serious issues to management as appropriate. -Monitor and review patient care and/or treatment progress using the Patient Tracking List in line with Trust and local policies. - Ensure the timely and efficient transfer of patients and information between trusts/other organisations involved in the patient pathway, adhering to agreed communication pathways and protocols. -Develop working relationships with a wide range of clinical and managerial staff. -Working with the Team to analyse target breaches and report issues to management. Person specification Qualifications Essential criteria Good general education with English and Maths to GCSE standard or equivalent Educated to diploma level or NVQ 3, City and Guilds certificate level in business administration or equivalent Evidence of some formal further training in short courses, experience of a range of admin practices and with significant experience of hospital administration Desirable criteria Customer services qualification/training IT package qualification/training, e.g. European Computer Driving Licence (ECDL) Experience Essential criteria Significant proven experience of delivering to high standards in a fastpaced high volume customer care environment. Ability to deal with a range of issues independently and efficiently Proven administrative experience Desirable criteria Experience of supervising staff Experience of an administrative role within the NHS Skills Essential criteria Ability to work both autonomously and in a team Ability to deal with difficult service users and challenging situations Demonstrable experience of data collection, presentation, report writing and handling of data quality issues Ability to work with confidential data and information and deal with sensitive issues discreetly Ability to follow complex protocols and feedback to senior staff as appropriate Strong IT skills, particularly with relation to Microsoft Office packages, e.g. Word, Outlook and Excel Desirable criteria Knowledge of medical terminology Understanding of peer review process and governance processes Guy’s and St Thomas’ celebrates, respects and values the diversity of its staff and patients.
