Hours
Permanent, part-time 20 hours per week. The service operates 7 days per week, 365 days per year, between 7:00am and 10:00pm. Successful applicants will work a flexible rota pattern across these operating hours, including weekends and bank holidays where required, with a minimum of 2 rest days.

Salary
£31,980 (pro rata) with enhancements for weekend and bank holiday hours worked (if applicable)

About the Role
The London Borough of Tower Hamlets is seeking to recruit five enthusiastic and motivated Reablement Officers to join our Adult Social Care Reablement Service. This is an exciting opportunity for experienced care workers who are passionate about promoting independence and supporting people to achieve their goals. You will become part of our therapy-led reablement service, helping residents regain confidence, function and independence following illness, injury, hospital admission or a reduction in their ability to manage daily activities.

You will bring the core skills and experience of a care worker, including the ability to provide hands-on support when required to meet duty of care responsibilities and support the achievement of reablement goals. Your focus will be on enabling residents to transition from dependence on care services to greater self-care, confidence and independence.

We welcome applicants who are able to speak one or more community languages commonly used in Tower Hamlets, including Bangla, Sylheti, and Somali.

Working as part of a multidisciplinary team, you will deliver person-centred reablement programmes in people's homes. In this role, you will:
- Deliver planned reablement programmes that support individuals to achieve their identified goals and maximise independence.
- Work closely with residents, families, carers and professionals to implement outcome-focused support plans.
- Support people to regain confidence with daily living activities such as personal care, meal preparation, medication routines, domestic tasks and community access.
- Promote self-care, strength-based practice and rehabilitation approaches rather than providing long-term care.
- Monitor and record progress against agreed outcomes and contribute to reviews of support plans.
- Maintain accurate, timely and confidential electronic records.

Experience
- Experience working within a health, social care, home care, residential care, rehabilitation or community support setting.
- Experience working with older people, adults with physical disabilities, frailty, dementia or long-term health conditions.

- Excellent spoken and written English skills with the ability to produce accurate records and communicate effectively with residents, carers and professionals.
- Comfortable working independently in the community and as part of a team.
- Willingness to undertake training and continuous professional development.
- Ability to speak one or more community languages commonly used in Tower Hamlets is welcomed.

Qualifications
- Level 2 qualification in Health and Social Care, or a Care Certificate.
