We offer a competitive salary with enhancements for weekend working The opportunity to work from home and skip the commute We offer a generous annual leave allowance We have a very supportive team and supportive managers We offer access to a lease car scheme run by the NHS Fleet Solutions We will provide a laptop and mobile phone We can offer subsidised leisure passes Local Government pension scheme We provide regular supervision and an annual appraisal scheme We will provide an Oyster Card for you to use when out in the community We offer genuine career progression Our Social Workers have support from our specialist practice development officers and the workforce development team What You Will Do Care Act assessments – you will carry out Care Act assessments with Adults who have been discharged from hospital Support plans – you will complete and review care and support plans for Adults who have been discharged from hospital Mental Capacity assessments- you will complete MCA with Adults either in hospital or in the community Work creatively with Adults, their families and carers- you will using a strengths based approach and evidence based practice with a good understanding of risk assessments and positive risk taking make the best use of available resources to enable people to have better lives and remain as independent as possible Arranging hospital discharges – in conjunction with health colleagues support Adults to remain as independent as possible, maintain their wellbeing, manage risk and participate in the life of their community You will work with those that self neglect or hoard What You Need Social Work qualification SWE registration 2 years post qualifying experience Excellent communication skills Empathy Critical thinking skills Self awareness Cultural competence This is a full time role for 36 hours a week. Due to the nature of the work staff work weekends on a rota basis with time off in lieu and enhanced pay for hours worked at weekends. What Next ?
