About the Role
Are you an experienced social worker looking for a different way to use your expertise? The London Borough of Merton's Targeted Review Team offers an opportunity to apply your professional skills in a role focused on reviewing, evaluating and improving support arrangements for adults receiving services across Adult Social Care. This role allows you to continue making a meaningful difference while working in a position that is less crisis-driven and more focused on quality, reflection, partnership working and service improvement.

As a Social Worker within the Targeted Review Team, you will work across Adult Social Care services undertaking reviews of individuals receiving support. Your work will help ensure services remain person-centred, proportionate and aligned with assessed needs, while promoting independence and positive outcomes for residents and carers.

Using your professional knowledge and analytical skills, you will:
- Complete strengths-based reviews and reassessments
- Evaluate support arrangements against assessed needs and outcomes
- Make evidence-informed recommendations regarding care and support provision
- Work collaboratively with residents, carers, providers and multidisciplinary colleagues
- Support effective decision-making and resource allocation
- Ensure care and support plans accurately reflect needs, strengths and goals
- Contribute to quality assurance and continuous service improvement

This position enables you to continue using many of the skills developed through frontline social work, including assessment and review, critical analysis and professional judgement, risk assessment and decision-making, professional curiosity, communication and stakeholder management, partnership working, and influencing outcomes and service delivery. The role offers a different professional focus, with less emphasis on managing complex statutory caseloads, safeguarding investigations and court-related work.

The Targeted Review Team plays an important role in ensuring Adult Social Care services remain effective, responsive and person-centred. You will have opportunities to influence service quality and practice standards, apply your expertise across a range of service areas, develop knowledge in review, quality assurance and service improvement, build strong relationships across multidisciplinary teams, and support improved outcomes through evidence-based decision-making.

Experience
Qualified and registered social workers with experience within Adult Social Care. Experience in assessment, analytical and decision-making skills is essential.

Qualifications
Registered Social Worker qualification required.
