About the Role
We are seeking passionate individuals to join our Leaving Care Service to provide a high quality and effective service for young people in our care as they transition to independence. As a Social Worker, you will conduct assessments, develop care plans, provide guidance and support, and advocate for the needs and rights of young people aged 16 and 17. This case holding role involves managing statutory duties, ensuring comprehensive and person-centred pathway plans are developed prior to young people leaving our care and transferring to a Personal Advisor within the service. You will play a key role in helping young people develop the skills, confidence, and resilience needed to live independently and achieve their aspirations. The service is undergoing exciting developments to offer the best wrap-around support for young people moving to independence. The successful candidate will be required to be office based at least 3 days a week.

Requirements

Experience
Experience in safeguarding and working with young people in care and leaving care services is essential. Knowledge of relevant legislation, policies, and procedures related to leaving care and young people in care services is required.

You will be passionate about making a positive impact and empowering young people to achieve their full potential. You should be committed to providing practical and emotional support, able to advocate for young people’s rights, and work collaboratively with colleagues and external agencies to provide holistic support.

Qualifications
Social Work qualification and Social Work England registration are required. Appointment to this post will be subject to an Enhanced Disclosure through the Disclosure and Barring Service (DBS).
