Description
Guy’s and St Thomas’ is among the UK’s busiest and most successful NHS foundation trusts. Evelina London cares for local children in Lambeth and Southwark and provides specialist services across south east England including cardiac, renal and critical care services. We lead a number of specialist service networks aiming to ensure children are treated locally where possible, but have access to specialist expertise when they need it. Our community services include health visiting, school nursing and support for families of children with long-term conditions.
Our adult community services teams deliver care at the heart of the local communities we serve, working in partnership with GPs, local authorities and other healthcare and voluntary sector organisations. Working with our partners in Lambeth and Southwark, we are focusing on new ways of working to improve care for local patients.

Job overview
An exciting opportunity has arisen to be a part of the Guy’s and St Thomas’ Community Tobacco Dependence Treatment Team. We are looking for an experienced Referral Coordinator on a full-time basis, who will enjoy working as part of a small team that delivers tobacco dependence treatment to clients across Guys and St. Thomas hospitals and different locations in Lambeth. The post holder will be part of the referral coordinators team, liaise with incoming service referrals via different pathways, participate in any admin tasks as required, and contribute to the service development and participation on local health improvement initiatives and outreach events
The post holder will be expected to familiarise themselves with the delivery of the smoking cessation sections of the Trust initiatives aimed at promoting and improving the health of local residents
Main duties of the job
Duties and responsibilities will include:
To provide administrative support to the Tobacco Dependence Treatment (TDT) Service at GSTT, including both the acute team and the local TDT team, also known as Lambeth Stop Smoking Service
Serve as primary contact for incoming referrals via different pathways (freephone, email, Epic database)
To ensure incoming referrals are processed in a timely manner and are referred onto the appropriate service.
To coordinate the development of administrative processes and maintenance of data collection systems to ensure that all steps on the patient pathway are booked and recorded appropriately
To ensure that all providers have access to the resources and equipment they need, managing stock and ordering supplies as needed.
Support the planning, coordination and delivery of community events and campaigns as required.
Contact patients via telephone and discuss referral pathway options in an open and informative manner

Detailed job description and main responsibilities
To liaise with referrals received by the acute team and action them accordingly
To work in a flexible manner providing an effective and comprehensive referral pathway for local residents that want to access support to stop smoking.
To conduct motivational interviewing over the telephone to help the smoker explore any conflict around changing the behaviour, identify gaps between smoking and other life goals and support the choice to quit.
To respond to telephone and email requests and enquiries from members of the public who wish to use the service
To be responsible for the ordering and payment of products and services via the GSTT procurement system.
Take minutes during team meetings, referral coordinators meetings, and other meetings when required
To take part in quality service improvement actions and contact previous clients for quality service feedback purposes
Contribute to the marketing and publicity of the smoking cessation services. Attend all necessary events to promote smoking cessation, including events that take place out of office hours and during weekends.
Support mechanisms for the planning, delivery and evaluation of the service.
Ensure consistent working methods and administrative systems to enable adequate back up during periods of sickness and annual leave

A qualification at NVQ level 3 or higher
