About the Role
This is a Team Leader based role where you will join our dedicated team providing support to vulnerable adults living independently within their own flats. You will be responsible for supporting individuals to live as independently as possible, engaging within their local community. Additionally, you will lead and manage a small team, ensuring excellent person-centred support, good quality assurance and compliance, and adherence to MTVH policies and procedures. The service supports 7 customers, each with unique learning disabilities and individual support requirements, focusing on maintaining independent living and achieving social inclusion. You will work flexibly over a Monday to Sunday rota, including bank holidays, with no sleep-in or waking night responsibilities at this time.

Experience
Experience of delivering support to vulnerable individuals with learning disabilities, autism, Asperger's syndrome, mental health issues, and/or complex needs. Experience in creating, reviewing, and implementing support plans and risk assessments. Experience in risk management.

About you
Excellent communication, writing, and reporting skills. Effective staff management and leadership skills. Strong IT skills are required as all records are maintained on a central IT system. Ability to conduct reports within deadlines and monitor customers throughout the shift. Ability to problem-solve and de-escalate situations, and establish mutually beneficial relationships with customer support networks.
