About the Role
As a Community Occupational Therapist, you will provide specialised rehabilitation assessment, education, advice, and treatment to adult patients in the community with complex health and social care needs. You will prioritise factors involved in patients' presenting conditions through clinical reasoning, considering current professional opinion, prognosis, and other physical, social, emotional, and mental health issues. You will set patient-centred treatment goals in collaboration with patients and/or their carers, ensuring care is agreed upon and tailored to individual needs. You will independently screen and assess multi-disciplinary referrals from various sources to establish priority, determine needs, and initiate intervention plans. Managing a caseload of patients with diverse needs, you will work under supervision and guidance from senior staff as necessary. Your work will take place across a variety of community locations including patients' homes, nursing homes, and residential homes, providing intensive rehabilitation. You will monitor, evaluate, and review occupational therapy treatments within a multi-disciplinary framework, modifying interventions as needed and taking a key worker role when indicated. You will ensure accurate recording and maintenance of patient and service records on IT databases in accordance with Trust and local policies. Additionally, you will prescribe, delegate, and monitor the work of rehabilitation assistants and counter-sign documentation. You will actively contribute to service development with support from senior therapy staff and participate in continuous professional development, education, training, audit, and research projects. Maintaining and updating a Continuing Professional Development Portfolio (CPD) is also required.
Experience
Experience in community occupational therapy or rehabilitation is desirable. Ability to manage a diverse caseload and work independently with clinical reasoning is essential.
