We are now looking for a warm, enthusiastic, and dedicated individual to join our award-winning team as a Dementia Team Leader at Wandsworth Common Care Home in London.

Your focus as Team Leader will be to support the home management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff.

Key responsibilities include:
- Leading the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations
- Completing care plan audits and developing action plans, with responsibility for completing these plans
- Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping residents' next of kin fully informed where appropriate
- Monitoring the well-being of each resident, being aware of any changes impacting care and care planning, and ensuring that changing needs are met with updated written records
- Practicing safe systems of work across a range of tasks including moving and handling residents, assessing risks, and ensuring the safety of residents, visitors, and staff

Experience
Minimum of two years' experience working in a senior position within a care home environment, specifically with the resident group.

Qualifications
NVQ Level 3 or equivalent qualification in care for care homes is required.
