Your role will involve delivering specialist knowledge and experience to support both the wider team and residents in overcoming personal barriers.

You will deliver evidence-based interventions based on holistic assessments to support personalised recovery, integration, and promotion of independent living skills. As a specialist in comorbidity, you will support the team to achieve positive outcomes for residents. Key duties include:

- Supporting multi-disciplinary and wider community teams with advice, signposting, and move-on care planning

- Ability to provide specialist support in risk assessment and risk management, particularly relating to harm minimisation, substance misuse, and mental health

About You

You will be committed to promoting recovery, harm reduction, and active involvement in care planning.

Additional qualities include:
- Full knowledge of comorbidity and complex needs, with the ability to share knowledge and skills with others
- Alignment with our values of Ambition, Empowerment, Inclusivity, and Transparency
