About the Role
We are seeking a dedicated Housing First Support Worker to join our passionate team on a fixed term contract until 31.03.2027, with potential for extension. You will support the delivery of our local strategy to end long-term rough sleeping by providing wrap-around intensive, trauma informed support to people with multiple and complex needs within their own independent tenancies in the community. Your role will involve maintaining accurate records on case management systems, liaising with a wide range of agencies including Adult Social Care, Criminal Justice, Health, and employment services, and regularly reviewing person-centred support plans. You will also build and maintain networks to promote the Housing First pilot and provide temporary cover for colleagues as required to ensure service availability across varied hours.

Requirements

Experience
- High level of experience supporting people experiencing multiple disadvantages including homelessness, substance misuse, mental and physical ill health, and offending behaviour
- Effective liaison with social, health, housing and criminal justice agencies
- Experience writing client records, completing monitoring materials and contributing to project evaluation
- Significant understanding and practice of risk and needs assessment, planning, goal setting and reviewing
- Ability to engage clients who may have low interaction or suspicion of services and build positive relationships
- Experience working with clients presenting challenging behaviour using a trauma informed approach

- Understanding of the complex interdependencies of mental health, substance abuse and offending behaviour
- Creative problem solver with empathy and ability to keep people engaged
- Advocate for clients promoting rights, responsibilities and informed choice to support independence
- Ability to seek assistance or supervision appropriately and engage meaningfully in supervision
- Adaptable, flexible and resilient to changing and emerging needs
- High level of relationship skills and ability to work collaboratively with multi-agency professionals
