About the Role
Tower Hamlets’ vision is to enable young people with Special Educational Needs and Disabilities (SEND) to live, learn and work within their local community, achieve sustained progression, and experience better life outcomes. We are seeking committed and proactive Preparation for Adulthood (PfA) Officers to join our SEN service. In this role, you will support young people as they transition from school or college into adult life, focusing on planning and ensuring their Education, Health and Care Plans (EHCPs) reflect their aspirations and that appropriate support is in place to realise these goals.
You will work closely with young people, their families, schools, colleges, social care, and health professionals. Your responsibilities include attending and contributing to annual reviews from Year 9 onwards, embedding PfA outcomes such as employment, independent living, community participation, and good health into each plan. You will promote access to opportunities like supported internships, apprenticeships, and training, and collaborate with professionals to facilitate smooth transitions into adult services where necessary.
This role requires excellent communication skills to explain complex processes clearly and sensitively, alongside empathy, patience, and a strong belief in the potential of every young person. Strong organisational skills and the ability to manage competing priorities are essential, as is confidence in working across multiple agencies.

Experience
Experience of transition planning and knowledge of the SEND Code of Practice are essential. Experience working with young people with SEND and multi-agency collaboration is highly desirable.
