About the Role
We are seeking one Social Care Officer to join the neighbourhood team within Adult Social Care. This role involves developing and applying skills in case work under the Care Act, including promoting independence and Direct Payments, carrying out assessments of needs, conducting reviews, and working closely with customers and carers in the community. You will also build strong links with community agencies to support effective care delivery. The ideal candidate will have good assessment and care planning skills and be passionate about delivering positive outcomes for service users.

Experience
