We are seeking an Advanced Practitioner Occupational Therapist to join Brent’s Reablement Team—an integrated service combining fast-paced responses to Home First discharges with reablement and social care services delivered through a short-term intervention model. The team promotes and enables independence and maintains strong partnerships with hubs and intermediate care services.

This role is ideal for an experienced Occupational Therapist with a background in working with adults with physical disabilities and older people. You will provide specialist advice, guidance, and supervision to non-qualified staff (OT Assistants) and work directly with Brent residents in their homes, conducting OT assessments and delivering reablement interventions.

Requirements

Experience
Proven experience working with adults with physical disabilities and older people in social care or community-focused multidisciplinary settings. Experience supervising staff and/or students, and working with Home Care or Reablement providers is desirable. Postgraduate experience in Social Care and/or Intermediate Care is preferred.

About you
Excellent communication skills and a strong understanding of moving and handling equipment. Confident in recommending minor and major adaptations. Passionate about supporting vulnerable adults and committed to delivering high-quality services. Strong multitasking, prioritisation, caseload management, and time management skills are essential.

Qualifications
Relevant professional qualification such as a Degree or Diploma from the College of Occupational Therapists (or equivalent). Current registration with the Health and Care Professions Council (HCPC) is mandatory. Evidence of recent and relevant Continuing Professional Development (CPD) is required.
