Description

Evelina London cares for local children in Lambeth and Southwark and provides specialist services across south east England including cardiac, renal and critical care services. We lead a number of specialist service networks aiming to ensure children are treated locally where possible, but have access to specialist expertise when they need it. Our community services include health visiting, school nursing and support for families of children with long-term conditions.
Our adult community services teams deliver care at the heart of the local communities we serve, working in partnership with GPs, local authorities and other healthcare and voluntary sector organisations. Working with our partners in Lambeth and Southwark, we are focusing on new ways of working to improve care for local patients.

Job overview
We are seeking an enthusiastic, full time, Nurse Specialist to join the Tuberculosis (TB) Team.
We are looking for someone forward-thinking and highly motivated when managing patients throughout their entire treatment.
You must demonstrate excellent communication and team working when liaising with other health professionals on a daily basis, as you will be working closely with the TB Consultants in all aspects of the patient’s care.
You will also receive support from the Nurse Lead, who you will also be able to discuss your training and development opportunities.

Main duties of the job
You will acts as case manager for your own caseload, co-ordinating and providing care within the community (including visiting patients at home).
You will maintain proactive and effective partnerships with patients and their families, acting as patient advocate, ensuring a patient orientated approach to the delivery of care according to the standards within the NHS Plan, and other relevant guidelines.
You will provide expert nursing care to adult and children infected with and affected by Tuberculosis by assessing, planning, implementing and evaluating skills to provide individualised care independently.
You will be supporting patients (adult and children) in adhering to difficult treatment regimens by assessing and developing strategies to overcome barriers to adherence.
You will ensure that patients’ receive high quality clinical care and a good patient experience, having regard for their customs, religious beliefs and doctrines.
You will also direct, support, encourage and advise clinical teams, by providing specialist knowledge and advice in the care of patients within the area
Working for our organisation
The Community Tuberculosis (TB) Team LSL (CTT LSL) works across Lambeth, Southwark, and Lewisham to provide a multidisciplinary service for clients who are marginalized and have difficulty accessing primary care services and providing care to clients with known or potential Tuberculosis e.g. refugees and asylum seekers, homeless clients, and clients with addictions.
We provide centred individualised case management for our patients throughout their entire treatment following them up periodically in our Nurse led clinic, monitoring their specific blood test, CXR’s, medications, side effects to the treatment, social issues that may affect their treatment, mental and emotional status that can also affect their adherence to their TB or latent TB treatment

Duties and Responsibilities

Acts as case manager for own caseload (London TB Target 1-Nurse to 40 patients (Adult and children) and (NICE guidelines (2016 Revised), co-ordinating and providing care within the community (including visiting patients at home).
Maintain proactive and effective partnerships with patients and their families and acts as patient advocate ensuring their needs are met appropriately.
Support patients (adult and children) in adhering to difficult treatment regimens by assessing and developing strategies to overcome barriers to adherence.
Ensure that patients’ receive high quality clinical care and a good patient experience, having regard for their customs, religious beliefs and doctrines.
Recognise and avoid situations that may be detrimental to the health and wellbeing of the individuals.
Direct, support, encourage and advise clinical teams, by the provision of specialist knowledge and advice in the care of patients within the area.
Participate in the promotion of patient centred care, and establish and maintain a supportive relationship with the patient and their family during the period of assessment, treatment and after care.
Support and enable patients and carers to make informed decisions relating to their treatment and management.
Promote high quality clinical care, and assist in the development of the specialist service, through the use of evidence based practice and clinical standards.
