London, based with the Homeless Health team at Beckett House opposite St. Thomas's Hospital, with travel to Guy's Hospital as required.

Hours
Core hours Monday to Friday 09:00-17:00, with one weekend per month.

About the Role
Guy’s and St Thomas’ NHS Foundation Trust is one of the UK’s busiest and most successful NHS trusts, providing a wide range of hospital and community services across south London. The Homeless Health Housing Worker plays a vital role in ensuring safe, appropriate, effective, and sustainable discharges for homeless patients. Working within a multidisciplinary team including housing workers, occupational therapists, nurses, and social workers, you will provide expert housing advice and support to patients and hospital staff. The role involves working on wards, in Accident and Emergency (A&E), and conducting outreach in the community to support patients post-discharge.

You will undertake detailed housing assessments to establish homelessness, eligibility, priority for housing, and local connection. Building strong relationships with Local Authorities’ Housing departments and other agencies is essential to achieve the best outcomes for patients. You may also escort patients to Housing Options departments and accommodation viewings to provide advocacy.

This role requires significant partnership working with NHS, housing, social care, and voluntary sector organisations to improve discharge processes and patient outcomes. You will contribute to teaching and awareness sessions for hospital staff and uphold the Trust’s values of caring, ambition, and inclusivity in all aspects of your work.

Requirements

Experience
- Direct work with homeless clients or clients with addictions
- Experience of case management
- Experience working in partnership and negotiating with a wide variety of agencies

About you
- Able to undertake holistic assessments for homeless clients
- Able to work effectively and constructively within a multidisciplinary team
- Able to manage conflict effectively
- Ability to write assessments and plans clearly and accurately
- Demonstrates sensitivity and delivers care without prejudice
- High-level communication skills to convey complex and sensitive information effectively, overcoming barriers such as language, literacy, stigma, and mental health conditions

Qualifications
- Educated to GCSE level with a pass in Maths or English (essential)
- Training relevant to housing legislation (desirable)
