Evelina London cares for local children in Lambeth and Southwark and provides specialist services across south east England including cardiac, renal and critical care services. We lead a number of specialist service networks aiming to ensure children are treated locally where possible, but have access to specialist expertise when they need it. Our community services include health visiting, school nursing and support for families of children with long-term conditions. Our adult community services teams deliver care at the heart of the local communities we serve, working in partnership with GPs, local authorities and other healthcare and voluntary sector organisations. Working with our partners in Lambeth and Southwark, we are focusing on new ways of working to improve care for local patients. In February 2021 the Royal Brompton and Harefield joined Guy’s and St Thomas’ NHS Foundation Trust, bringing together world-leading expertise in the care and research of heart and lung disease. Job overview The Patient Pathway Coordinator will lead on the tracking of patient pathways to help streamline the investigative and diagnostic processes for patients newly referred to Haematology on a suspected cancer pathway and throughout their care under our teams. The post holder will need to support and facilitate a co-ordinated patient experience focusing on improving the support and tracking of the patient through their diagnostics, treatment plans and investigations. By working closely with a wide range of clinical and managerial staff, and providing high standard of administrative support, the post holder will proactively ensure haematology patients move along their pathways as quickly as possible. The post holder will be the main point of contact for the patients to ensure that their appointments, diagnostics and treatment occur within national cancer targets and in the best interests of the patients. The post holder is responsible for providing a high standard of professional and patient focused service to a number of sub-specialty malignant haematology specialities and will support the creation and management of a one-stop clinic model. Main duties of the job Working with the cancer MDT co-ordinator to monitor and review patient care and/or treatment progress using the Patient Tracking List in line with all national cancer waiting times targets. The post holder will support the 2WW Appointment Officers and will liaise with Departmental Managers and senior clinicians regarding capacity for outpatient appointments and diagnostic testing, to ensure all patients are seen within the 2 week standard, whilst aiming for a 7 day standard. Work with the MDT Leads, MDT Co-Ordinators, General Managers and Service leads to ensure capacity is maintained to enable the cancer patients to meet the 28 day Faster Diagnosis target whilst still ensuring the 14, 38 and 62 day targets are met. Liaise with patients to ensure required tests are performed in advance of clinics and proactively ensure results are received and loaded onto trust systems prior to clinic and chase missing investigations. Receive and send onward patient referrals and administer in line with timings and requirements as set out in Trust and National policies. Co-ordinate and provide administrative support to the relevant teams to ensure that all the relevant paperwork and clinical information is available. Develop a knowledge and understanding of the patient pathway for individual tumour sites. Develop and maintain up to date knowledge of the standards and targets being set nationally and locally for cancer. Detailed job description and main responsibilities Escalate any issues and breaches of the Waiting Times standards to the relevant management in line with agreed escalation procedures. Ensure the timely and efficient transfer of patients and information between trusts/other organisations involved in the patient pathway, adhering to agreed communication pathways and protocols. Undertake designated actions from each clinic ensuring that requested actions have been completed and that documentation is available for specific patients (e.g. consent forms, chemo info). Validation of outpatients lost to follow up reports Work within a multi-disciplinary team in developing the service in line with departmental plans and Trust corporate objectives. Participate in national audits and collect data as required in relevant meetings, allowing the Trust to identify and improve treatment in patient cohorts. Responsible for collecting and updating patient information on the relevant databases in line with local policy, to ensure efficient pathway progress. This includes maintaining the accuracy of data at all times. Working with the Team to analyse cancer target breaches and report issues to management. Review and support development and implementation of Standard Operating Procedures. Review and support development and implementation of internal targets Undertake any administrative tasks commensurate with the grade required to support the delivery of high quality patient care. Actively participate in department development as appropriate and participate in departmental meetings Undertake any other duties commensurate with the grade, required to ensure the smooth and efficient running of the department. Undertake any training as required to complete the tasks associated with the job role. Person specification Qualifications/ Education Essential criteria Good general education with English and Maths to GCSE standard or equivalent Educated to A-Level or diploma level or NVQ 3, City and Guilds certificate level in business administration or equivalent Evidence of some formal further training in short courses, experience of a range of admin practices and with significant experience of hospital administration Desirable criteria IT package qualification/training, Customer services qualification/training Skills/Knowledge/Ability Essential criteria Excellent interpersonal and communication skills face to face, in writing and on the telephone with a wide range of multi-disciplinary staff and patients.
