Shifts between 08:00 and 20:00 including weekend working. Occasional shifts with the unified point of access (UPA) admin team depending on service needs.

About the Role
Guy’s and St Thomas’ Trust is one of the UK’s busiest and most successful NHS foundation trusts, providing a wide range of hospital and community services across south London and specialist care for patients from further afield. The @Home team is an accelerator site for Urgent Community Response across South East London, delivering intensive multidisciplinary acute clinical care at home for patients in Lambeth and Southwark who would otherwise require hospital admission.
We are seeking an enthusiastic and motivated Patient Pathway Coordinator to join the @Home team on a permanent basis. This role involves managing clinical visit allocations, reviewing workforce capacity daily, performing administrative duties, and monitoring patient pathways within the service.
The role requires excellent organisational and prioritisation skills to work effectively in a busy environment, strong interpersonal and communication skills to liaise with multi-disciplinary teams and patients, and proficient IT skills, particularly in Excel and data management. The successful candidate will be proactive in identifying and suggesting improvements to patient pathways.
The @Home service supports early discharge from hospitals and hospital admission avoidance through rapid response working closely with multiple healthcare partners including Guy’s & St Thomas, King’s College, London Ambulance Service, GPs, Community Matrons, District Nurses, Social Care, and community therapy teams.

Experience
- Proven administrative experience
- Experience in allocating work to clinical staff or booking patient appointments
- Good general customer service experience and/or working with clinical colleagues
- Desirable: experience in an NHS administrative role, supervising staff, and using EPIC or other health systems
About you
- Excellent interpersonal and communication skills with the ability to engage effectively with a wide range of multi-disciplinary staff
- Strong organisational skills with the ability to plan, prioritise and manage your own workload and that of junior staff
- Proactive and motivated with a keen eye for detail and a commitment to improving patient pathways
- Strong IT skills including data collection, presentation, report writing, and maintaining data quality
Qualifications
- Good general education with English and Maths to GCSE standard or equivalent
- Educated to diploma level or NVQ 3, City and Guilds certificate level in business administration or equivalent
