About the Role
Guy’s and St Thomas’ NHS Foundation Trust is one of the UK’s busiest and most successful NHS trusts, providing a wide range of hospital and community services across south London and specialist care for patients from further afield. The @home team is a Hospital at Home service covering Lambeth and Southwark boroughs, delivering intensive multidisciplinary acute clinical care at home that would otherwise be provided in hospital.
We are seeking an enthusiastic and motivated Patient Pathway Coordinator for a six-month secondment to join the expanding @home team. This role involves managing clinical visit allocations, reviewing workforce capacity daily, performing administrative duties, and ensuring patient needs are met, particularly for those requiring care packages delivered by our nursing rehab support worker workforce.
The role includes occasional shifts with the @home unified point of access (UPA) admin team, involving call handling, processing referrals, checking patient demographics, analysing data, and other administrative tasks.
The successful candidate will work closely with a multidisciplinary team including consultants, GPs, nurses, therapists, social workers, and pharmacists, supporting early discharge and hospital admission avoidance initiatives in partnership with local healthcare and social care providers.

Experience
- Proven administrative experience
- Experience in allocating work to clinical staff or booking patient appointments
- Good general customer service experience and/or working with clinical colleagues
- Desirable: experience in an NHS administrative role, supervising staff, and using EPIC or other health systems

- Strong interpersonal and communication skills to work well with a wide range of multidisciplinary staff
- Proactive and motivated with the ability to identify and suggest pathway improvements
- Strong IT skills, including data collection, presentation, report writing, and data quality management
Qualifications
- Good general education with English and Maths to GCSE standard or equivalent
- Educated to diploma level or NVQ 3, City and Guilds certificate level in business administration or equivalent
