Description
Guy’s and St Thomas’ is among the UK’s busiest and most successful NHS foundation trusts. We provide a full range of hospital and community services for people in south London and as well as specialist care for patients from further afield including cancer, renal, orthopaedic, respiratory and cardiovascular services.
Guy’s is home to the largest dental school in Europe and a £160 million Cancer Centre opened in 2016. As part of our commitment to provide care closer to home, in 2017 we also opened a cancer centre and a kidney treatment centre at Queen Mary’s Hospital in Sidcup. St Thomas’ has one of the largest critical care units in the UK and one of the busiest emergency departments in London. It is also home to Evelina London Children’s Hospital.
Evelina London cares for local children in Lambeth and Southwark and provides specialist services across south east England including cardiac, renal and critical care services. We lead a number of specialist service networks aiming to ensure children are treated locally where possible, but have access to specialist expertise when they need it. Our community services include health visiting, school nursing and support for families of children with long-term conditions.
Our adult community services teams deliver care at the heart of the local communities we serve, working in partnership with GPs, local authorities and other healthcare and voluntary sector organisations. Working with our partners in Lambeth and Southwark, we are focusing on new ways of working to improve care for local patients.
In February 2021 the Royal Brompton and Harefield joined Guy’s and St Thomas’ NHS Foundation Trust, bringing together world-leading expertise in the care and research of heart and lung disease. Job overview
Our Population Health Equity Programme has built strong foundations at a system-wide level, sharing learning and facilitating connections across organisational boundaries to explore different ways to help our diverse communities. A key component of this is the Vital 5 Programme: improving population health and reducing health inequalities through tackling the five key drivers of health inequality in SEL: high blood pressure, obesity, alcohol consumption, smoking, and common mental health conditions. The post holder will be responsible for managing the development and delivery of distinct projects and initiatives in relation to the wider Population Health & Equity Programme.
The post holder will be responsible for designing and leading projects to develop the overall programme, including supporting other team members in the development, implementation and monitoring of work streams that support the delivery of the overall programme. Performance management and cross-system reporting of the programme is of vital importance, given the complex stakeholder environment. The post holder will be responsible for ensuring all work streams are reporting effectively within the overall governance and reporting framework.
Main duties of the job
Responsible for the development, delivery and implementation of projects across the local health system including building support from boards, senior managers, and potential funders of the importance of the programme
Coordinate all projects across the programme to ensure implementation of the projects and report on progress to the Programme Board and other governance structures as appropriate.
Develop recommendations; gain support, implement and performance manage services relevant to the implementation of the projects ensuring ‘best value’ and an outcome-based approach.
Develop and maintain excellent working relationships with all stakeholders, internal and external to ensure the projects are implemented effectively and efficiently.
Build support of wide range of organisations across the health system by managing complex mix of senior stakeholders, developing approaches to ensuring that senior leaders are informed, engaged and involved as appropriate in the development and delivery of the programme.
Lead engagement with wider networks at sector, regional, national and international levels to facilitate dissemination of the evidence developed through the programme and to secure external funding
Use improvement methods within the project and programme plans, and ensure appropriate monitoring arrangements are in place.
It is expected that the specific programme / projects managed by the post holder will change over time as the portfolio of work changes.

Detailed job description and main responsibilities
Introduction
Our population in south-east London (SEL) is one of the most vibrant and ethnically, socially, and economically diverse in the world. However, we face significant challenges in respect of both health and healthcare, some of which are familiar to other urban areas and some of which are unique to SEL. Whilst there have been some successes in these areas which we are rightly proud of, there remains:
there remains a high burden of disease (both physical and mental);
unacceptable health inequalities (both between and within boroughs); and
quality of care is variable and does not always meet the needs of the most disadvantaged or those with the greatest need.
We have a bold aspiration in SEL, for every resident to be able to live a happy and healthy life in an equitable and equal society. We want SEL to be a place where your postcode, background or ethnicity is not be the most powerful predictor of future health.
To support this aspiration, we have established a ‘Population Health and Equity Programme’. This is joint programme between SEL ICS and KHP. The aim of the programme is to reduce health inequalities in the local population, put in place the necessary structures and mechanisms to ensure long-term improvements in health outcomes and to make population health an integral part of how the health and care sector operates.
To support our collective aim of improved health and wellbeing in south east London whilst addressing health inequalities, we have three priority areas (described on page 1). The programme manager role will primarily be focussing on:
Prevention, Wellbeing and Health Equity: Have a relentless focus on the small number of things that disproportionately impact health and health inequalities, starting with the Vital 5 (tobacco dependence, high blood pressure, alcohol consumption, obesity and common mental health conditions). This priority area aims to reduce inequalities in the detection, management & treatment of long-term conditions and fewer preventable deaths.
Population Health Equity Programme team
The core PH&E team in KHP relates closely to colleagues in other KHP teams and within the ICS. The postholder will play a key role in programme management in the Vital 5 Programme and will also support the development of other priority areas, as needed.
Role responsibilities:
Develop and maintain an up-to-date picture of SEL residents’ population health and inequalities, on the Vital 5 domains of hypertension, smoking, mental health, obesity, and alcohol.
Analyse complex data from varying sources, providing effective reports to a variety of stakeholders.
Support with impact and evaluation analyses of the programme and its initiatives.
Provide contemporary awareness of the availability, quality, and needs of SEL’s rich data sources and resources, including through NHS Business Intelligence teams, public health analysts, King’s College London, Clinical Effectiveness South East London (CESEL), Lambeth datanet, and the PHM Catalyst.
Develop and maintain effective working relationships with clinical and non-clinical teams, initiatives, and colleagues from across KHP and SEL ICS.
Update the team and key stakeholders on developments of Vital 5 initiatives.
Ensure integration of Vital 5 activity with the broader priority area initiatives (e.g. vaccinations, women’ and girl’s health.
Engage key stakeholders in the design and delivery of projects that support improved health outcomes. The key benefit will be to realise significant, sustainable and affordable improvements in the quality of service delivery that is ambitious and far reaching.
Take an active role in effective team working and supporting colleagues, taking ownership for team goals.
Effectively representing the team and organisation within the SEL system and externally as required, including through effective relationship building and communication.
Promote a culture that embraces equality and strives continuously to reduce inequality for all south-east London residents and staff.
Support the implementation of programme aims and in strategic and operational forward planning.
