Description
Guy’s and St Thomas’ is among the UK’s busiest and most successful NHS foundation trusts. Evelina London cares for local children in Lambeth and Southwark and provides specialist services across south east England including cardiac, renal and critical care services. We lead a number of specialist service networks aiming to ensure children are treated locally where possible, but have access to specialist expertise when they need it. Our community services include health visiting, school nursing and support for families of children with long-term conditions.
Our adult community services teams deliver care at the heart of the local communities we serve, working in partnership with GPs, local authorities and other healthcare and voluntary sector organisations. Working with our partners in Lambeth and Southwark, we are focusing on new ways of working to improve care for local patients.

Job overview
An exciting opportunity has arisen to be a part of the Guy’s and St Thomas’ Acute Tobacco Dependence Treatment Team. We are looking for two experienced Tobacco Dependence Specialists who will enjoy working as part of a small team delivering tobacco dependence treatment to patients across our two hospital sites. The post holder will be involved in continuing the development and delivery of our services to meet the Long Term Plan for tobacco, including ward based interventions, outpatient clinics and MDT training. This will help to achieve the Trust commitment to drive a culture change towards prevention and to bring the promotion of optimised physical health and wellbeing into the mainstream.
Main duties of the job
The post holder will be expected to support the delivery of the tobacco dependence treatment sections of the Trust initiatives aimed at promoting and improving the health of patients including the LTP project and Vital 5 programmes.
Duties and responsibilities will include:
To ensure that staff in the target clinical areas are trained in very brief advice (VBA) either through online package or face to face training.
To monitor ward staff compliance with the core care, including screening on admission and initial withdrawal management, and further support staff with any issues resulting in non-compliance of this target
Provide appropriate clinical interventions to patients to support time-limited withdrawal management and long-term smoking cessation
To be responsible for delivery of the targets set.
To set up, manage and deliver tobacco dependence outpatient clinics
To be engaged and proactive with the further development and roll out of the tobacco dependence treatment programmes developed by the Trust throughout the hospital.

Detailed job description and main responsibilities
To assess inpatients tobacco dependence, devise a treatment plan and ensure tobacco dependence care is given throughout the inpatient stay and post-discharge
To uphold and actively support the Trust’s smoke-free status
Ensure targets are met
To deliver a range of effective tobacco dependence services using ward-based and out patient clinic-based support and provide telephone support for those smokers who are unable to attend face to face appointments.
To manage a caseload of clients independently and prioritise service user needs
Develop expertise in effective treatment programmes for people with a range of smoking related health conditions
Plan and deliver a comprehensive and bespoke training programme to promote a systematic, proactive approach to tobacco dependence across the Trust.
To utilise a range of different approaches to raise awareness of tobacco dependence services and to use all possible opportunities to generate referrals for patients
To promote and facilitate the development of seamless care pathways and referral systems between professionals in acute settings, midwifery, community health services and primary care.
To support the integration of the tobacco dependence service into broader health development initiatives
To contribute to the ongoing development, delivery, monitoring and evaluation of the tobacco dependence treatment service

Two years’ experience of working within Tobacco Dependence Treatment or other patient facing clinical role, in an acute setting and delivering 1-1 interventions.
Experience of working on a target driven programme eg LTP
Experience of caseload management
Experience of developing and delivering training for a range of health professionals and others
Experience of setting up Tobacco Dependence Treatment Clinics
Experience of audit and evaluation
Desirable criteria
Experience of working with multidisciplinary teams in an acute setting
Qualifications
Essential criteria
Educated to degree level
Stop Smoking Practitioner Training (accredited by the National Centre for Smoking Cessation and Training) Level2 training
