Description
Guy’s and St Thomas’ is among the UK’s busiest and most successful NHS foundation trusts. Evelina London cares for local children in Lambeth and Southwark and provides specialist services across south east England including cardiac, renal and critical care services. We lead a number of specialist service networks aiming to ensure children are treated locally where possible, but have access to specialist expertise when they need it. Our community services include health visiting, school nursing and support for families of children with long-term conditions.
Our adult community services teams deliver care at the heart of the local communities we serve, working in partnership with GPs, local authorities and other healthcare and voluntary sector organisations. Working with our partners in Lambeth and Southwark, we are focusing on new ways of working to improve care for local patients.

Transfer of Care Navigator - George Perkins Ward
An exciting opportunity has arisen for a 12 month maternity leave cover for a Transfer of Care Navigator on George Perkins Ward.
George Perkins Ward is a 28 bedded trauma orthopedic ward which is one of London’s busiest trauma admission wards. This is a fast paced clinical environment for patients with orthopedic emergency trauma cases. The patients would have orthopedic emergency with complex discharge needs. The successful candidate will need to have effetcive communication skills as a large aspect of this role liasing with the on site teams and community colleagues to ensure safe and supported discharged for all our patients.
The successful candidate will need to be efficient and able to work well independently. They will take accountability for ensuring all patient discharges are completed in a safe and timely manner. You will be expected to actively participate in multidisciplinary meetings and board rounds, passionately advocating for our patients. They will need to be confident in escalating any concerns early and ensuring patients, their families and carers are involved in all aspects of the discharge planning process.
Main duties of the job
To risk assess and identify complex discharges on admission, and in collaboration with the multidisciplinary team set discharge dates to assist the Trust in meeting it’s NHS access targets.
To work in collaboration with a dedicated multidisciplinary team, Social Services and other relevant personnel in both the hospital and community to plan and manage timely and appropriate transfers of care (discharge) for patients from the hospital.
Act as an advocate for patients discharge, to ensure a patient orientated approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines.
Attend and participate in multi-professional team meetings to facilitate safe discharges.

Detailed job description and main responsibilities
The successful candidate will be expected to have outstanding communication skills as they will need to communicate frequently with community services including district nurses, social services and the discharge hub.
A key part of this role is ensuring appropriate referrals have been completed and confirmed prior to the patients being discharged.
