About the Role
Guy’s and St Thomas’ NHS Foundation Trust is one of the UK’s busiest and most successful trusts, providing a full range of hospital and community services across south London and specialist care nationally. Evelina London Children’s Hospital, part of the Trust, delivers specialist paediatric services and community care across Lambeth and Southwark. We are excited to offer a unique 18-month fixed-term opportunity (open to existing GSTT employees) to join the Children’s Community Physiotherapy Team as a Children’s Physical Activity Advisor, with potential to become substantive.

This innovative role focuses on supporting children and young people with physical and learning disabilities to access inclusive sport and physical activity opportunities within their communities. Taking an autonomous, family-centred approach, you will empower families to build sustainable, healthy activity habits by exploring interests, identifying barriers, and setting achievable goals to increase confidence and participation.

You will act as a community connector, building partnerships with local sports clubs, leisure centres, schools, and community providers to link families with appropriate opportunities. The role involves working independently across clinics, homes, schools, and community settings while collaborating with colleagues across health, education, and community sectors.

This is a rare chance to shape a role with lasting impact, promoting inclusion and community-based healthcare for children and young people with disabilities.

Main Duties
- Work autonomously to improve access to inclusive sports and physical activity for children and young people with physical disabilities
- Support development of sustainable physical activity habits as part of self-management and healthy lifestyles
- Assess individual needs, interests, and barriers to participation
- Collaborate with children and families to set and achieve physical activity goals

- Assist children and families in increasing participation in community activities and identify additional support needs
- Promote inclusive practice and accessibility through collaboration with local providers and professionals
- Support transition from physiotherapy-based exercise to community activity
- Maintain accurate patient records and documentation using EPIC electronic system
- Plan, organise, and prioritise own caseload and work as a lone practitioner in various community settings
- Communicate effectively with multidisciplinary teams, referrers, and external agencies
- Undertake other duties as required to ensure smooth running of the role and service
