About the Role
Are you a passionate Occupational Therapist who wants to make a real difference in the lives of young people as they move into adulthood? We are seeking an enthusiastic and motivated Occupational Therapist to join our Transitions Programme. This role offers a rare opportunity to influence life-changing outcomes by working alongside young people and families at a pivotal stage of life, supporting their journey towards greater independence, employment opportunities, community inclusion, and successful adult living.

You will work with young people aged 14 to 25 who have a range of disabilities including learning disabilities, autism, physical disabilities, neurodevelopmental conditions, or complex health needs. Collaborating with professionals across Adult Services, Children and Family Services, Health, and Education, you will support young people and their families through the transition from Children's and Family Services to Adult Services, ensuring empowerment to achieve greater independence, choice, and meaningful participation in everyday life.

This unique role spans both children's and adult pathways, allowing you to build long-term relationships and influence transition planning from age 14 through adulthood. You will provide occupational therapy assessments, interventions, and care coordination, helping individuals develop the skills and confidence needed for adulthood.

Working collaboratively with social care, health, education, housing, and voluntary sector partners, you will promote positive outcomes in areas such as independent living, daily living skills, behavioural interventions, education, training and employment, community access and participation, housing and accommodation, assistive technology and adaptations, sensory needs, and wellbeing.

You will contribute to person-centred, strengths-based transition planning focused on achieving meaningful outcomes. Supported by an experienced Lead Occupational Therapist, you will benefit from regular supervision, reflective practice, and professional development opportunities.

Experience
- Relevant post-qualification experience in an Occupational Therapy role.
- Experience of occupational therapy assessment and intervention in health, social care, or community settings.
- Experience working with children, young people, or adults with learning disabilities, autism, physical disabilities, mental health needs, or complex needs.

Qualifications
- Degree or recognised qualification in Occupational Therapy.
- Registration with the Health and Care Professions Council (HCPC).
- Knowledge of relevant legislation related to occupational therapy and social care.
